I understand that this can be done at about $1.00 per page from a
commercial establishment. Alternatively, I believe that Canon makes
a machine to do this for about $15000.

If I had access to the hard copy, lots of volunteer time, I would buy
a high resolution, professional type digital camera, probably a
NIKON/KODAK, photograph each page, and download it to a RW CD-ROM.

Hardware cost would be much less than Canon's prices, resolution and
flexibility would be higher, and you could do it in color if you had
color originals.
